2024-2025 NRP Passport Request

Completion of this form does not guarantee approval of a NRP Passport.  OMHA will review and approve all NRP Passports, not West Lorne Minor Hockey as per OMHA Regulation 3.5: Exemption to Play at Another Centre.

This submission completes the condition of "registering with your home centre" as required by OMHA Regulations.

WLMHA Registrar will submit your NRP Passport Request to OMHA. If approved, the NRP Passport will then be distributed to the player/parent/guardian (whose email address is associated with this HCR Playerès Profile), the issuing Association, the Association with which the player will be trying out, as well as, the OMHA Office.

Should a player return to play for West Lorne Minor Hockey, a separate registration will be required.
