Grand Valley Minor Hockey 2024/2025 Season

Terms & Conditions:

 

PAYMENT MUST BE MADE IN FULL BY DECEMBER 1ST 2024, UNLESS OTHER ARRANGEMENTS HAVE BEEN MADE AND APPROVED. REGISTRATIONS FOR GRAND VALLEY MINOR HOCKEY ASSOCIATION ARE SUBJECT TO APPROVAL FROM OUR EXECUTIVE.

 

Any requests for refunds of registration must be in writing. All refunds granted after hockey operations for the season have commenced will be subject to a non-refundable administration charge of $25.00 and will be pro-rated based on the hours of ice-time provided to the player.

a- No refund will be issued until any equipment belonging to GVDMHA is returned in acceptable condition.

b- No partial refunds will be issued due to illness or injury for part of the season.

c- No request for refunds will be accepted after December 1st of the current season, unless the family moves or unless there are medical reasons.

 

I/My participating child herby agree to abide by and support the current Ontario Minor Hockey Association Code of Conduct, and rules, regulations and decisions of the OMHA (Ontario Minor Hockey Association), OHF (Ontario Hockey Federation), HC (Hockey Canada) and GVDMHA (Grand Valley and District Minor Hockey Association).  The OMHA Code of Conduct identifies the standard of behavior, which is expected of all OMHA, for the purpose of this policy participants shall include all GVDMHA players, parents, guardians, coaches, officials, volunteers, directors, officers, committee members, convenors, team managers, trainers, administrators and employees.

 

GVDMHA is committed to providing an environment in which all individuals are treated with respect.

 

Members and participants of GVDMHA shall conduct themselves at all times in a manner consistent with the values of OMHA and GVDMHA, which includes fairness, integrity and mutual respect.  During the course of all OMHA and GVDMHA activities and events, members shall avoid behavior, which brings OMHA, GVDMHA or the sport of hockey into disrepute, including but not limited to the use of alcohol, cannabis and non-medical drugs.

 

I, as a member and participant of GVDMHA, shall not engage in any activities or behaviours which interfere with a competition or with any player or team’s preparation for a competition or which endanger the safety of others.

 

I, as a member and participant of GVDMHA, shall refrain from comments or behaviours, which are disrespectful, offensive, abusive, racist, or sexist.  In particular, behavior, which constitutes harassment or abuse, will not be tolerated and will be dealt with under OMHA’s Harassment Policy and GVDMHA By-laws.

 

I, as a member or participant have read and understood the OMHA SOCIAL MEDIA POLICY  

 

I herby understand and appreciate that participation as a hockey player carries risks to me or my participating child of serious injury, including permanent disability, paralysis, or death.  I/My participating child voluntarily and knowingly acknowledge, accept and assume these risks.

 

Failure to comply with these Terms and Conditions may result in disciplinary action in accordance with the Discipline Policy of the OMHA and GVDMHA By-laws and Polices.  Such action may result in the member losing the privileges that come with membership in GVDMHA and OMHA, including the opportunity to participate in GVDMHA and OMHA activities and events, both present and future.

 

I the participant/parent have become subject to the rules, regulations and decisions of the OMHA, OHF, HC and GVDMHA.  I am aware that these rule and regulations are available to me through my local association and certify that the provided information is true

 

I signify my agreement to the above term and conditions when I register my child

 

MEDICAL FORM

 

As with all seasons, all players/participants shall have an updated medical form filled out for the 2024/2025 Season.

 

Filling out this form now will greatly assist us in being proactive for rostering of the season. Please fill out the form and hand it to  the team’s trainer once the season starts.
