2024-25 Chedoke Minor Hockey House League Registration

I understand that it is my responsibility to ensure any and all payments are completed on or before the due dates provided. 

Failure to complete payments may result in removal of the player from the program.

NSF payments will be subject to an administration fee.

Refund Policy You must complete the Refund Request form to receive a refund. All refunds are pro-rated as follows:

Form received on or before Refund = Amount paid less August 31 $ 25 September 30 $ 50 October 31 $135 November 30 $185 December 31 $235 There are no refunds after December 31. 1. Advising a coach, convenor or any other person at Chedoke Minor Hockey of your wish to withdraw is not considered a formal refund request and will not be processed as such. 2. The date used to determine the refund amount is the date this form is received. 3. Additional fees may be withheld if uniforms have been given out and are not returned. 4. All refunds will be issued in the form of payment, credit card, cheque etc. 5. Please allow 3 to 4 weeks for processing. 6. Refunds due to injuries received during play with CMHL will be assessed on an individual basis. A doctor's note may be required.
