Acknowledgements and Waiver

1. I/We hereby declare that the information provided in this online registration is true to the best of my/our knowledge. Warren Park Hockey reserves the right to request proof of this information, including but not limited to birth certificate and proof of residence, to confirm registration. Failure to provide such proof may result in the application being denied and/or registration voided.

2. By filling out this application, you agree to release and hold harmless Warren Park Hockey, their legal representatives, advertisers, sponsors and each of their respective parents, subsidiaries, affiliates, directors, officers, employees, volunteers and agents from any and all liability for any injuries, loss or damage of any kind to the player, and his or her parent/guardian arising from or in connection with participation in this league, whether or not such damages are foreseeable

3. Warren Park Hockey reserves the right to accept or reject any Application/Registration Form and acceptance is determined at the sole and absolute discretion of the officers of Warren Park Hockey and may not be disputed.

4. Any player or parent/guardian not conducting themselves in a sportsmanlike manner, or disrespecting or abusing directors, officers, coaches, managers, officials, volunteers, other players or parents/guardians are subject to disciplinary action, including suspension or ejection from the league. If disciplinary action is required, the penalty will be determined at the sole and absolute discretion of the officers of Warren Park Hockey and may not be disputed. IF SUSPENSION OR EJECTION IS DEEMED FITTING, ENTRY FEES ARE NOT REFUNDABLE.

5. The placement of players will be determined at the sole and absolute discretion of the officers of Warren Park Hockey and may not be disputed. The placement of players will be based on age, skill level and availability. In order to balance the teams, players may be required to move to a different team or division after the start of the season.

6. Players registered in the Warren Park Hockey may not play in any other sanctioned registered league unless a release form is granted and vice versa as per the rules of the GTHL, NYHL, OHF and Hockey Canada.

7. All players participating in the Warren Park Hockey are covered by Hockey Canada's accident insurance. In order to ensure coverage, proper equipment, including CSA-approved helmets and CSA-approved neck guards, must be worn by all players at all times on the ice and on the bench.

8. There is no bodychecking allowed in Warren Park's house league nor on Warren Park's Select Teams that represent Warren Park Hockey in the NYHL.

9. Scheduling, location and length of games is the sole responsibility of the Warren Park Hockey Executive. It may be necessary to modify schedules and game lengths due to time constraints, and other factors.

Privacy Policy

I/We consent and grant permission to Warren Park Hockey to collect and utilize the personal information of the Participant provided above to administer and conduct the Program and to collect and use Participant’s name, voice, statements, photograph, image, likeness, actions at the Program in any live or recorded form (including, but not limited to, any form of video display or other transmission or reproduction), in whole or in part, for promotional, commercial or any other purpose, in perpetuity worldwide on standard and non‐standard television, home video, print, electronic and online media (including, without limitation, social media), and in any other means of distribution, publication or exhibition, whether now known or hereinafter created without any additional consideration in connection with the Program, future programs, and the marketing, advertising and promotion thereof.
